- For new patients please complete the New Patient forms below. Please make sure to provide all information requested and upload relevant documents that will help us understand your child’s therapy history. If you have any difficulty with any of these forms, feel free to call us and we can walk you through the process.
- All patients are expected to have an active credit card on file.
- Please arrive on time for your child’s scheduled appointment. If you have not arrived within 5 minutes, we will give you a call. Any late cancellations or missed appointments will incur a “late cancel/no-show” fee as indicated in the signed Attendance Policy.
