Contact UsThe evaluating therapist will meet with you and your child at the beginning of the evaluation session to gather patient information relevant to treatment, and to discuss any parent concerns. Once all relevant information is received, the therapist begins the evaluation process, which may be standardized, function-based, or a combination of the two depending on your child's needs. At the end of the evaluation, the therapist will discuss initial findings with you, send a copy to your referring physician once documentation is complete, and make a copy available to you upon request.
Generally, your doctor will write a referral for therapy. At your initial appointment, bring your child's referral as well as their insurance card. You'll be asked to fill out some paperwork before your child is seen — see our New Client Forms for more detail.

Yes — for your convenience, new client forms can be completed prior to your visit to save you time. Contact our office and we'll get those to you right away.
It depends on your insurance plan — some require a physician referral before starting therapy, while others don't. Our office is happy to help you check your specific requirements, and can work directly with your child's doctor to get any needed referral on file.
